Athletic Registration
CIF requires athletes to have an athletic physical on file before athletes begin the first day of practice/weights on August 27th.
This is mandatory, 2-step paperless process. It must be done each year!
Your athlete will NOT be cleared to participate until the required waivers and agreements
are signed. A valid, dated, signed physical must be uploaded.
Step 1: Take this FORM to the Doctor for your physical exam. Form must be dated and signed by physician!
Step 2: Register your athlete HERE for the 2024-2025 school year. You will find two links, one to REGISTER and one link to UPLOAD the Medical Eligibility Form (signed and dated physical form). You must do both to be cleared for participation.
