Athletic Registration
CIF requires athletes to have an athletic physical on file before athletes begin the first day of practice/weights.
This is mandatory, 2-step paperless process. It must be done each year!
Your athlete will NOT be cleared to participate until the required waivers and agreements
are signed. A valid, dated, signed physical must be uploaded.
Step 1: Take this FORM to the Doctor for your physical exam. Form must be dated and signed by physician!
Step 2: Register your athlete HERE for the 2025-2026 school year. You will find two links, one to REGISTER on SNAP (add Medical Eligibility form here) and a second google link to once again UPLOAD the Medical Eligibility Form for the Health Office. You must do both to be cleared for participation.
